Target Audience
This activity is intended for primary care physicians, specialty physicians (Internal Medicine, Ob/Gyn), Physician Assistants, Nurse Practitioners, Nurses, Care Coordinators, Advanced Clinical Practitioners, and Care Managers.
Statement of Need
Lack of knowledge in the understanding the social, demographic, and medical risks that put patients at risk for high blood pressure, the guidelines for initiating medications or other evidence-based treatments, understanding low-pressure approaches to treatment, including non medical approaches and lack of awareness of self-measured blood pressure monitoring with clinical support
Educational Objectives
Upon completion of this activity learners should be able to:
- Recognize the social and demographic factors contributing to hypertension challenges in the U.S. and Delaware.
- Describe evidence-based interventions to improve hypertension control.
- Describe hypertension guidelines and how to utilize them in your practice.
