Target Audience
This activity is intended for primary care physicians, specialty physicians (Internal Medicine, Ob/Gyn), Physician Assistants, Nurse Practitioners, Nurses, Care Coordinators, Advanced Clinical Practitioners, and Care Managers.
Statement of Need
Lack of education on evidence based programs that can assist providers in their efforts to prevent or control type 2 diabetes within their patient population..
Educational Objectives
Upon completion of this activity learners should be able to:
1. Describe the prevalence diabetes and prediabetes in Delaware.2. Identify at least two programs and/or resources available in Delawareans living with diabetes and prediabetes.
3. Identify two ways in which health literacy can affect diabetes outcomes.
